If you were unable to find the driver of a motor vehicle that YOU or SOMEONE who was utilizing your vehicle hit, whether major or minor damage, use this report form to report the accident. The Mesa Department Of Motor Vehicle will seek to find the driver's insurance information and provide that to our agency so we can find that person and notify them of the accident. (NOTE: If you hit someone, regardless if someone is around or not, you should always stay at the scene of the accident as it is considered a felony leave the scene. However, this report will not lead to any hit and run charges if you are reporting in good faith and have attempted to contact the driver or driver's family to swap information.)
WHAT YOU WILL NEED:
Your Drivers License Number
Your Insurance Company's Identifier Information
A copy of your vehicle title(If you can not locate your vehicle's title, request one and it will be sent to you[Title records cost about 250.00/ each.)
A completed Self-claim Accident Report(This form can be found here(coming soon)
SOME INFORMATION THAT WOULD BE HELPFUL TO HAVE IF POSSIBLE:
The victim driver's license plate number, vehicle make, model and year, color, and VIN #(This number can be found in the left hand corner of the windshield.)
The victim driver's license number(again, if available)
The drivers insurance company identifier and other information.
The driver's address.
